NNRTI stands for Non-nucleoside reverse transcriptase inhibitor, a drug class abbreviation.
HIV drugs that bind directly to reverse transcriptase at a site away from the active center, including efavirenz, nevirapine, rilpivirine, doravirine and etravirine. They are combined with two NRTIs.
Example: Switch to NNRTI-based regimen (doravirine) to avoid INSTI weight gain.
Usage notes
NNRTIs have a low genetic barrier: a single mutation can confer resistance to several. Rilpivirine must be taken with a meal and cannot be combined with proton pump inhibitors. Nevirapine can cause severe liver toxicity, especially in women with higher CD4 counts at the start of treatment. Long-acting rilpivirine is part of the injectable Cabenuva regimen.
