CategoriesMedical

NK1 RA stands for Neurokinin-1 receptor antagonist, a drug class abbreviation.

Antiemetics, including aprepitant, fosaprepitant, rolapitant and netupitant, that block substance P at NK1 receptors. They are added to a 5-HT3 antagonist and dexamethasone to prevent nausea from highly emetogenic chemotherapy such as cisplatin.

Example: Cisplatin day 1: fosaprepitant 150 mg IV (NK1 RA), ondansetron, dexamethasone.

Usage notes

Aprepitant (Emend), approved in 2003, was the first in class and is especially effective against delayed nausea occurring days after treatment. It inhibits CYP3A4, so dexamethasone doses are reduced when combined. NK1 antagonists also prevent postoperative nausea.
