Health plan with a network of contracted providers that offers lower costs in-network but still pays part of the cost of out-of-network care, and usually lets members see specialists without referrals. Premiums are generally higher than for HMOs in exchange for this flexibility.
Example: PPO members pay a $30 copay in-network or 40% coinsurance after the deductible out-of-network.
PPOs are the most common plan type among US workers with employer coverage, according to annual KFF surveys. Out-of-network providers can bill the difference between their charge and the plan's allowed amount (balance billing), although the No Surprises Act limits this for emergencies and certain hospital-based services. Pronounced as letters.
2. Proximal policy optimization

A reinforcement learning algorithm that improves an agent's policy in small, clipped steps so training stays stable. It became a default RL algorithm for many tasks and was used in OpenAI's RLHF pipeline for InstructGPT and ChatGPT.
Example: Tune the PPO clip range if the reward curve keeps collapsing mid-training.
Published by John Schulman and colleagues at OpenAI in 2017 as a simpler alternative to trust region policy optimization (TRPO). It also trained OpenAI Five, the Dota 2 system. In US health insurance, PPO means preferred provider organization, a far more common meaning.
