1. Patient-controlled analgesia

Medical · Medication Route Abbreviations

Pain-control method in which a programmable pump lets the patient press a button to self-administer small IV opioid doses, within limits set by the prescriber: dose, lockout interval and hourly maximum, with or without a continuous basal rate.

Example: Hydromorphone PCA 0.2 mg demand, 10-minute lockout, no basal, 4-hour max 4 mg.

The Joint Commission issued a Sentinel Event Alert in 2004 on "PCA by proxy", where family members press the button for a sleeping patient, causing respiratory depression. Continuous capnography or pulse oximetry is recommended. ISMP notes that "PCA" has also been used for procainamide, a dangerous confusion. PCA additionally means posterior cerebral artery.

2. Prostate cancer (PCa)

Medical · Nephrology & Urology Abbreviations

Cancer of the prostate gland, the most common cancer in American men apart from skin cancer, diagnosed in roughly one in eight men during their lifetime. Many tumors grow slowly; low-risk disease is often managed with active surveillance, while higher-risk disease is treated with surgery, radiation and hormone therapy.

Example: Low-risk PCa, grade group 1, PSA density 0.09; enroll in active surveillance.

The lowercase "a" distinguishes it from other PC abbreviations. Grading uses the Gleason score, simplified since 2014 into ISUP grade groups 1 to 5. Risk is higher in Black men and those with a family history or BRCA2 mutations. Prostate cancer is a leading cause of cancer death in men.

3. Principal component analysis

Units & Measurements · Statistics Abbreviations · also used in technology

A technique that transforms correlated variables into a smaller set of uncorrelated components ordered by how much variance they explain. It is used for dimensionality reduction, visualization and noise reduction.

Example: The first two PCA components explained 68 percent of the variance in the gene-expression data.

Karl Pearson introduced the idea in 1901 and Harold Hotelling developed it in 1933. PCA is computed from the eigenvectors of the covariance matrix or through singular value decomposition, and variables are usually standardized first because the method is sensitive to scale. It differs from factor analysis, which models latent causes.

4. Patient care assistant

Medical · Healthcare Credential Abbreviations

Hospital job title for unlicensed staff who help nurses with bathing, toileting, ambulation, vital signs and room turnover. The role is similar to a nursing assistant or patient care technician.

Example: PCA obtained 0400 vitals and assisted patient to the bathroom.

Training and required certification vary by hospital; many require CNA status. Tasks are delegated by registered nurses, who remain accountable. In pharmacy and nursing PCA more commonly means patient-controlled analgesia, a pump for self-administered pain medication.

5. Personal care aide

Medical · Healthcare Credential Abbreviations

Home and community care worker who helps older adults and people with disabilities with daily activities such as bathing, dressing, meals and housekeeping. Many are funded through Medicaid home- and community-based services.

Example: Medicaid waiver approved 20 hours/week of PCA services.

Training requirements are set by states and are often lighter than those for home health aides. Some programs let consumers hire family members as their PCA. The workforce is mostly women and has faced low wages and high turnover.

6. Posterior cerebral artery

Medical · Anatomy Abbreviations

Paired terminal branch of the basilar artery that supplies the occipital lobes, the undersides of the temporal lobes and parts of the thalamus and midbrain. A PCA stroke typically causes loss of vision in the opposite half of the visual field (homonymous hemianopia).

Example: Left PCA infarct: right homonymous hemianopia and alexia without agraphia.

In a minority of people the PCA fills mainly from the internal carotid through a large posterior communicating artery, a variant called a fetal PCA. Central vision is often spared in PCA strokes because the visual cortex for the macula has a dual blood supply. In anesthesia and pain management PCA means patient-controlled analgesia.

7. Permanent Court of Arbitration

Law & Legal · Court Abbreviations · also used in organizations

Intergovernmental organization at the Peace Palace in The Hague, founded by the 1899 Hague Convention, that administers arbitrations and conciliations between states, state entities, international organizations and private parties. Despite its name it has no sitting judges; tribunals are formed case by case.

Example: The investor-state dispute was administered by the PCA under the UNCITRAL Arbitration Rules.

It administered the 2016 South China Sea arbitration between the Philippines and China and many investment treaty cases. It is the oldest standing institution for international dispute resolution. In medicine PCA means patient-controlled analgesia, and in statistics principal component analysis.
