Umbrella term for chronic immune-mediated inflammation of the gut, chiefly Crohn's disease and ulcerative colitis. Diagnosis rests on colonoscopy with biopsies, cross-sectional imaging and stool markers such as fecal calprotectin; therapy ranges from aminosalicylates and steroids to biologics including anti-TNF agents, vedolizumab and ustekinumab.
Example: New IBD referral: bloody diarrhea for six weeks, calprotectin 820 mcg/g, colonoscopy scheduled Thursday.
Pronounced as letters. IBD is often confused with IBS; the key difference is that IBD produces visible inflammation, ulcers and tissue damage. When features of Crohn's and colitis overlap, the label IBD-U (IBD unclassified) is used. Incidence is highest in North America and northern Europe and rising in newly industrialized countries. Extraintestinal manifestations include arthritis, uveitis, erythema nodosum and primary sclerosing cholangitis.
