IPAA stands for Ileal pouch-anal anastomosis, a gastroenterology abbreviation.
Restorative operation after total proctocolectomy in which the end of the small intestine is folded into a reservoir and joined to the anus, avoiding a permanent stoma. It is the standard elective surgery for ulcerative colitis and familial adenomatous polyposis.
Example: Status post IPAA for UC in 2018; now with pouchitis, starting ciprofloxacin for 14 days.
Usage notes
The reservoir is usually called a J-pouch because of its shape. Pouchitis, an inflammation of the pouch, affects up to about half of patients at some point. It is usually performed in two or three stages with a temporary loop ileostomy. Crohn's disease is generally a contraindication.
