CategoriesMedical

HAS-BLED stands for Hypertension, Abnormal kidney or liver function, Stroke, Bleeding, Labile INR, Elderly, Drugs or alcohol, a cardiology abbreviation.

Score estimating the risk of major bleeding in patients with atrial fibrillation on anticoagulants, with one point for each factor and a maximum of 9. A score of 3 or more marks high bleeding risk, prompting attention to modifiable factors rather than automatically withholding anticoagulation.

Example: HAS-BLED 3 due to uncontrolled HTN and NSAID use; stop ibuprofen and recheck BP in 2 weeks.

Usage notes

Published in 2010 by Pisters and colleagues using Euro Heart Survey data. Guidelines stress that a high score is a reason to correct modifiable risks, such as blood pressure, alcohol and unnecessary antiplatelet drugs, and to follow the patient more closely, not to deny stroke prevention. The "labile INR" item applies only to warfarin users.
