HFmrEF stands for Heart failure with mildly reduced ejection fraction, a cardiology abbreviation.
Heart failure with an ejection fraction of 41 to 49 percent, a middle category between HFrEF and HFpEF. Patients often resemble the reduced-EF group and may benefit from some of the same medications, especially SGLT2 inhibitors.
Example: Repeat echo shows LVEF 45%, now classified as HFmrEF; continue SGLT2 inhibitor and beta-blocker.
Usage notes
Originally named heart failure with mid-range ejection fraction in the 2016 European guidelines; "mildly reduced" replaced "mid-range" with the 2021 Universal Definition. Many such patients have recovering or deteriorating systolic function, so serial echocardiograms are used to follow the trend rather than relying on a single reading.
