GDMT stands for Guideline-directed medical therapy, a cardiology abbreviation.
Medications and doses recommended by current practice guidelines for a condition, most often used in heart failure to describe the full set of proven drugs titrated to target doses. The term also appears in coronary disease, valve disease and device eligibility criteria.
Example: Hold ICD decision until 3 months of GDMT, then repeat the echo.
Usage notes
GDMT for heart failure with reduced ejection fraction now includes four drug classes, and registries show many patients are undertreated or never reach target doses. Device guidelines often require about 90 days of GDMT before an ICD or CRT is implanted for primary prevention. The phrase was popularized by ACC/AHA guidelines in the 2010s; "OMT" (optimal medical therapy) is an older equivalent.
