CategoriesMedical

POEM stands for Peroral endoscopic myotomy, a gastroenterology abbreviation.

Incisionless endoscopic procedure for achalasia in which a tunnel is created inside the esophageal wall and the circular muscle of the lower esophageal sphincter is cut. It relieves dysphagia comparably to surgical Heller myotomy, though reflux afterward is more common.

Example: Type II achalasia on manometry; scheduled for POEM next month.

Usage notes

First performed in humans by Haruhiro Inoue in Japan in 2008. Gastric POEM (G-POEM) applies the same idea to the pyloric muscle for refractory gastroparesis. Pronounced like the word "poem". Patients often take a proton pump inhibitor afterward because of reflux risk.
