1. Primary biliary cholangitis

Autoimmune disease that slowly destroys the small bile ducts inside the liver, mostly in middle-aged women. Diagnosis rests on a raised alkaline phosphatase and antimitochondrial antibodies; ursodeoxycholic acid is first-line, with obeticholic acid or newer PPAR agonists for inadequate responders.
Example: AMA positive, ALP 2.5 times normal, fatigue and pruritus: PBC, start ursodiol 13–15 mg/kg/day.
Renamed in 2015 from primary biliary cirrhosis, because most patients diagnosed today never develop cirrhosis and the old name carried stigma. Antimitochondrial antibodies are found in about 95 percent of cases. Elafibranor and seladelpar received accelerated FDA approval in 2024 as second-line treatments.

PBC status is a legal form and differs from B Corp certification, which is awarded by the nonprofit B Lab.
