1. Peptic ulcer disease

Breaks in the lining of the stomach or duodenum deep enough to penetrate the muscularis mucosae, caused mainly by Helicobacter pylori infection and NSAID or aspirin use. Symptoms include burning epigastric pain and dyspepsia; complications are bleeding, perforation and gastric outlet obstruction.
Example: History of PUD with prior bleed; avoid ibuprofen and continue pantoprazole while on aspirin.
"Peptic" refers to pepsin and acid, once thought to be the whole explanation. The 2005 Nobel Prize in Physiology or Medicine went to Barry Marshall and Robin Warren for showing that H. pylori causes most ulcers, turning PUD from a chronic surgical disease into a curable infection. Gastric ulcers (GU) are biopsied to exclude cancer, whereas duodenal ulcers (DU) are almost never malignant.
