HOPI stands for History of presenting illness, a medical documentation abbreviation.
British and Commonwealth heading for the detailed account of the patient's current complaint, equivalent to the American HPI. It follows the presenting complaint (PC) in clerking notes.
Example: PC: chest pain. HOPI: central, tight, started 2 hours ago at rest, radiating to jaw.
Usage notes
Medical students in the UK commonly use the SOCRATES mnemonic (site, onset, character, radiation, associated symptoms, time course, exacerbating or relieving factors, severity) for the HOPI. Some writers use HPC, history of presenting complaint, instead. The overall clerking structure is otherwise very similar to the American H&P.
