Hx stands for History, a medical documentation abbreviation.
Shorthand for history, used alone or in combinations such as "Hx of" (has a history of) and in headings like PMHx and FHx. It refers to past conditions or the patient's reported story.
Example: Hx of recurrent UTIs and nephrolithiasis; no prior surgeries.
Usage notes
The "x" ending forms a family of medical shorthand: Dx diagnosis, Tx treatment, Rx prescription, Sx symptoms, Fx fracture and Bx biopsy. The x has no fixed meaning; it was borrowed by analogy with Rx. "Hx of" can sound like an active problem, so notes should state when an old condition resolved.
