AIN stands for Acute interstitial nephritis, a nephrology & urology abbreviation.
Inflammation of the kidney's interstitium and tubules, most often an allergic reaction to drugs such as proton pump inhibitors, NSAIDs, beta-lactam antibiotics and immune checkpoint inhibitors. Kidney function declines over days to weeks, and the main treatment is stopping the culprit drug, sometimes with corticosteroids.
Example: Rising creatinine 3 weeks after starting omeprazole, sterile pyuria; AIN suspected, PPI stopped.
Usage notes
The classic triad of fever, rash and eosinophilia occurs in only a minority of patients, so kidney biopsy is often needed. Urine eosinophils are no longer considered reliable. Infections and autoimmune diseases such as sarcoidosis are less common causes. Pronounced as letters.
