SIADH stands for Syndrome of inappropriate antidiuretic hormone secretion, an endocrinology & diabetes abbreviation.
Disorder in which ADH is released despite low blood osmolality, causing water retention and euvolemic hyponatremia with inappropriately concentrated urine. Causes include lung and brain disease, small-cell lung cancer, pain, nausea and drugs such as SSRIs, carbamazepine and cyclophosphamide.
Example: Sodium 124, urine osmolality 480, urine sodium 52, euvolemic: SIADH from sertraline, fluid restrict 1 L.
Usage notes
First described by Schwartz and Bartter in 1957. Many authors now prefer SIAD (syndrome of inappropriate antidiuresis), since some cases arise from kidney receptor mutations rather than hormone excess. Chronic hyponatremia must be corrected slowly, generally no more than 8–10 mEq/L in 24 hours, to avoid osmotic demyelination.
