Formatted with "related to" (cause) and "as evidenced by" (signs and symptoms), sometimes shortened to r/t and AEB. Risk diagnoses have no evidence statement because the problem has not happened yet. Most NDx labels come from NANDA International.

The Invesco QQQ Trust, launched in 1999, is the best-known fund tracking the index. Excluding financial companies distinguishes it from the broader Nasdaq Composite, which covers more than 3,000 stocks. The index is reconstituted annually in December.
