dMMR
Stands forMismatch repair deficient
CategoriesMedical

dMMR stands for Mismatch repair deficient, a hematology and oncology abbreviation.

Tumor that has lost function of one or more DNA mismatch repair proteins (MLH1, MSH2, MSH6 or PMS2), shown by absent staining on immunohistochemistry. It is the protein-level counterpart of microsatellite instability and identifies cancers likely to respond to immunotherapy.

Example: Endometrial cancer with loss of MLH1 and PMS2: dMMR, MLH1 promoter methylation test to exclude Lynch syndrome.

Usage notes

Universal testing of all colorectal and endometrial cancers is recommended to identify Lynch syndrome and guide treatment. Loss of MLH1 is often caused by acquired promoter methylation rather than inheritance, so further testing distinguishes sporadic from hereditary cases. Proficient tumors are labeled pMMR. The lowercase d stands for deficient.
