Databases that are shared, replicated and synchronized across many participants without a central administrator, using consensus rules to agree on updates. Blockchains are the best-known kind of DLT.
Example: The banks are testing DLT to settle bond trades in minutes instead of two days.
The term spread in finance and government from around 2015, partly as a neutral alternative to "blockchain", which was associated with bitcoin. Permissioned ledgers such as Hyperledger Fabric and R3's Corda restrict who can take part. Not every distributed ledger uses a chain of blocks; some use directed acyclic graphs.
2. Dose-limiting toxicity

Side effect serious enough to prevent increasing the dose of a drug in a phase 1 trial, usually defined in the protocol as specific grade 3 or 4 toxicities during the first treatment cycle.
Example: Grade 4 neutropenia lasting over 7 days counted as a DLT in cycle 1.
Toxicities are graded using the National Cancer Institute's Common Terminology Criteria for Adverse Events (CTCAE). The proportion of patients with DLTs at each dose level determines the maximum tolerated dose and the recommended phase 2 dose (RP2D).
