1. Dexamethasone

Medical · Drug Abbreviations

Clinical shorthand for dexamethasone, a potent, long-acting glucocorticoid used for brain swelling from tumors, croup, nausea prevention with chemotherapy, severe COVID-19 and many inflammatory conditions.

Example: Croup, barky cough: dex 0.6 mg/kg PO x 1, observe 2 hours.

In the RECOVERY trial in 2020, dexamethasone 6 mg daily reduced deaths among COVID-19 patients needing oxygen or ventilation, the first drug shown to improve survival in the disease. It has almost no salt-retaining effect. Because "dex" also means dexmedetomidine in ICUs, full names are safer in orders.

2. Decentralized exchange (DEX)

Technology & Computing · Cryptocurrency & Blockchain Abbreviations

A cryptocurrency exchange run by smart contracts, where users trade directly from their own wallets without depositing funds with a company. Uniswap, Curve and Jupiter are major DEXs.

Example: The token isn't listed on any big exchange yet, so buy it on a DEX and double-check the contract address.

Most use automated market makers with liquidity pools rather than order books. Because anyone can list a token, scam and copycat tokens are common. Pending trades are visible publicly before confirmation, exposing users to front-running and sandwich attacks.

3. Dexterity (DEX)

Texting & Internet Slang · Gaming Slang Abbreviations

Ability score measuring agility, reflexes and balance in Dungeons & Dragons and many RPGs, affecting ranged attacks, initiative, stealth and armor class.

Example: Rogues live and die by DEX, put your highest roll there.

Often considered one of the most useful scores because it boosts several systems at once. Many video-game RPGs, such as Dark Souls, also use a Dexterity stat for agile weapons. In pharmacology, "dex" can refer to dexamethasone, an unrelated meaning.

4. Dexmedetomidine

Medical · Drug Abbreviations

ICU and anesthesia shorthand for dexmedetomidine (Precedex), an alpha-2 agonist sedative infused for ventilated and non-ventilated patients. Patients remain easily rousable and breathing is largely preserved.

Example: Dex gtt 0.4 mcg/kg/h for agitation; RASS goal -1; HR 54, monitor.

Approved in the US in 1999, dexmedetomidine causes slow heart rate and low blood pressure, especially with loading doses. A sublingual film (Igalmi) was approved in 2022 for agitation in schizophrenia and bipolar disorder. The same "dex" nickname for dexamethasone has led to near-miss errors.
