1. Complete heart block
Medical · Cardiology Abbreviations
Third-degree AV block in which no atrial impulses reach the ventricles, so atria and ventricles beat independently (AV dissociation). A slow escape rhythm, often 20 to 40 beats per minute, keeps the patient alive, but fainting, low blood pressure and sudden death can occur.
Example: Found with HR 32 and P waves marching through unrelated QRS complexes: CHB, temporary transvenous pacer placed.
Treatment is a permanent pacemaker unless a reversible cause is found, such as drug toxicity, hyperkalemia or Lyme disease. Congenital CHB can occur in infants of mothers carrying anti-Ro/SSA antibodies, as in lupus or Sjögren syndrome. A narrow escape rhythm suggests a junctional source and is more stable than a wide ventricular escape.
