Oral drugs, palbociclib, ribociclib and abemaciclib, that halt the cancer cell cycle. Combined with hormonal therapy, they are standard first-line treatment for hormone receptor-positive, HER2-negative advanced breast cancer.

Example: Letrozole plus CDK4/6i ribociclib 600 mg days 1-21 of 28; ECG for QTc.

Usage notes

Palbociclib (Ibrance), approved in 2015, was the first. Neutropenia is the main side effect of palbociclib and ribociclib, while abemaciclib more often causes diarrhea.
