BTKi stands for Bruton tyrosine kinase inhibitor, a drug class abbreviation.
Oral targeted drugs that block a key signaling enzyme in B cells: ibrutinib, acalabrutinib, zanubrutinib and pirtobrutinib. They have transformed treatment of chronic lymphocytic leukemia, mantle cell lymphoma and Waldenstrom macroglobulinemia.
Example: CLL with del(17p): continuous BTKi zanubrutinib 160 mg BID.
Usage notes
Ibrutinib, approved in 2013, was the first. Side effects include bleeding, atrial fibrillation and high blood pressure; second-generation agents such as acalabrutinib and zanubrutinib cause fewer heart rhythm problems. Pirtobrutinib binds non-covalently and works in some patients whose disease has become resistant to other BTK inhibitors.
