BRUE stands for Brief resolved unexplained event, a pediatric & neonatal abbreviation.

Event in an infant under one year, lasting less than a minute and now resolved, involving one or more of: color change to blue or pale, absent or irregular breathing, marked change in muscle tone, or altered responsiveness, with no explanation after history and exam.

Example: Lower-risk BRUE in 3-month-old, 20-second pallor and limpness, normal exam; discharged with CPR teaching.

Usage notes

The AAP introduced BRUE in a 2016 guideline to replace ALTE. Lower-risk infants (older than 60 days, born at 32 weeks or later, first event, under one minute, no CPR by trained provider, normal exam) need little testing and usually no admission. Pronounced "brew".
