INSTI stands for Integrase strand transfer inhibitor, a drug class abbreviation.

HIV drugs that block integrase, the enzyme that inserts viral DNA into human chromosomes: raltegravir, elvitegravir, dolutegravir, bictegravir and cabotegravir. INSTI-based regimens are preferred initial therapy in most guidelines.

Example: Initial ART: INSTI bictegravir with TAF/FTC as a single tablet.

Usage notes

Raltegravir, approved in 2007, was the first. Dolutegravir and bictegravir rarely select for resistance. Long-acting cabotegravir injections are used every one or two months for treatment (with rilpivirine) and for HIV prevention (Apretude, approved in 2021). Weight gain is a recognized side effect, and antacids must be separated from doses.
