hs-cTn stands for High-sensitivity cardiac troponin, a cardiology abbreviation.
Generation of troponin assays able to measure very low concentrations, detectable in most healthy people, allowing heart attack to be ruled in or out within one to three hours of arrival. Results are reported in nanograms per liter and compared with the assay's sex-specific 99th percentile.
Example: hs-cTn 4 ng/L at 0 hours and 5 ng/L at 1 hour: MI ruled out under the 0/1-hour algorithm.
Usage notes
Accelerated pathways such as the ESC 0/1-hour algorithm let many chest pain patients go home safely from the emergency department. Because small elevations are common in kidney disease, heart failure and sepsis, a rising or falling pattern matters more than a single value. Assays target troponin I or troponin T, and values differ between manufacturers.
