CategoriesMedical

FOLFIRINOX stands for Folinic acid, fluorouracil, irinotecan, oxaliplatin, a hematology and oncology abbreviation.

Intensive four-drug chemotherapy regimen for pancreatic cancer in patients with good performance status, used for metastatic disease, after surgery and before surgery in borderline resectable disease. It is given every two weeks.

Example: Borderline resectable pancreatic cancer: mFOLFIRINOX x8 cycles, then restaging for surgery.

Usage notes

A 2011 French trial showed median survival of about 11 months versus 7 months with gemcitabine in metastatic pancreatic cancer, and the PRODIGE 24 trial (2018) showed a large survival benefit after surgery. Toxicity is substantial, so a modified version (mFOLFIRINOX) without the fluorouracil bolus is widely used. Growth factor support is often needed.
