DPP-4i stands for Dipeptidyl peptidase-4 inhibitor, a drug class abbreviation.
Oral diabetes drugs ending in "-gliptin", such as sitagliptin, linagliptin and saxagliptin, that prolong the action of the body's own incretin hormones. They lower blood glucose modestly without weight gain or much hypoglycemia.
Example: Older adult, eGFR 28: linagliptin (DPP-4i) 5 mg daily, no renal adjustment.
Usage notes
Sitagliptin (Januvia), approved in 2006, was the first. Linagliptin is notable for needing no dose change in kidney disease. The FDA has warned about severe joint pain, and the drugs are linked to bullous pemphigoid. They are not combined with GLP-1 receptor agonists because both act on the same pathway.
