C/D/I stands for Clean, dry and intact, a nursing abbreviation.

Dressing or site assessment indicating the dressing is free of drainage or soiling, has no moisture beneath it and remains sealed to the skin. It is charted for surgical incisions, IV and central line sites, drains and other dressings.

Example: Midline abdominal incision dressing C/D/I, no erythema at edges.

Usage notes

Also written CDI or "clean/dry/intact". If any part does not apply, the note should describe the problem, such as "small amount of serous drainage, edges peeling". The letters CDI also mean Clostridioides difficile infection and clinical documentation improvement, so the slashes help readers tell them apart.
