BTB stands for Bone-patellar tendon-bone (graft), a musculoskeletal & rheumatology abbreviation.
ACL reconstruction graft harvested from the central third of the patellar tendon with bone blocks from the kneecap and tibia at each end. The bone-to-bone healing in the tunnels is strong and fast, making it a long-standing choice for high-demand athletes.
Example: ACL reconstruction with ipsilateral BTB autograft, 10 mm, interference screw fixation.
Usage notes
Often called the historical "gold standard" graft. Drawbacks include anterior knee pain, pain with kneeling and a small risk of patellar fracture. Hamstring and quadriceps tendon grafts are common alternatives. Pronounced as letters.
