Medicare form (CMS-R-131) given to Original Medicare patients before a service Medicare may not cover, stating the expected cost so the patient can decide whether to proceed and accept financial responsibility. Without a valid ABN, the provider generally cannot bill the patient if Medicare denies the claim.
Example: Signed the ABN for the vitamin D test since Medicare may not consider it medically necessary.
The notice must be specific, delivered before the service and include a cost estimate. Typical uses are lab tests that may exceed frequency limits and services that may not meet coverage criteria. Medicare Advantage plans use different notices and pre-service coverage decisions.
