2-PAM stands for Pralidoxime, a drug abbreviation.
Antidote for poisoning by organophosphate pesticides and nerve agents such as sarin. Given with atropine, it reactivates acetylcholinesterase if administered before the enzyme bond becomes permanent ("aging").
Example: Organophosphate exposure: atropine 2 mg IV q5min to dry secretions plus 2-PAM 30 mg/kg.
Usage notes
Military and emergency responders carry DuoDote or ATNAA autoinjectors containing atropine and pralidoxime for nerve agent attacks. The time to aging varies by agent and is only minutes for soman. Evidence for pralidoxime in pesticide poisoning is mixed, but it remains standard in many guidelines.
