ISMN stands for Isosorbide mononitrate, a drug abbreviation.
Long-acting oral nitrate taken once or twice daily to prevent angina, the active metabolite of isosorbide dinitrate. It is not used for acute chest pain, which requires sublingual nitroglycerin.
Example: ISMN ER 30 mg every morning for stable angina.
Usage notes
Continuous nitrate exposure causes tolerance, so immediate-release ISMN is taken in asymmetric doses (for example 8 a.m. and 3 p.m.) to leave an overnight nitrate-free period, while ER versions are taken once in the morning. Headache is the most common side effect and often fades after the first weeks.
