iNO stands for Inhaled nitric oxide, a respiratory abbreviation.
Gas delivered through a ventilator or nasal cannula that relaxes blood vessels only in ventilated parts of the lung, lowering pulmonary pressures and improving oxygenation. It is approved for newborns with hypoxic respiratory failure and pulmonary hypertension.
Example: Term newborn with PPHN, oxygenation index 28: iNO started at 20 ppm.
Usage notes
The FDA approved iNO in 1999 for term and near-term infants. It is also used during right heart catheterization to test pulmonary vasoreactivity and after cardiac surgery or lung transplantation, while in adult ARDS it improves oxygen levels without improving survival. Abrupt withdrawal can cause rebound pulmonary hypertension, and methemoglobin levels are monitored.
