doxy-PEP stands for Doxycycline post-exposure prophylaxis, a drug abbreviation.

Single 200 mg dose of doxycycline taken within 72 hours after condomless sex to prevent bacterial sexually transmitted infections, mainly syphilis and chlamydia, and to a lesser degree gonorrhea.

Example: Offered doxy-PEP: doxycycline 200 mg within 72 h of condomless sex, max once daily.

Usage notes

The CDC issued guidelines in 2024 recommending doxy-PEP for gay and bisexual men and transgender women who had a bacterial STI in the past year, after the DoxyPEP trial showed roughly two-thirds fewer infections. Trials in cisgender women did not show benefit. Concerns include antibiotic resistance, especially in gonorrhea and Staphylococcus aureus.
