CDTM stands for Collaborative drug therapy management, a pharmacy abbreviation.
Arrangement under state law that allows pharmacists to initiate, modify or stop drug therapy under a written protocol with physicians, commonly for anticoagulation, diabetes, hypertension and lipid management.
Example: CDTM anticoagulation clinic adjusted warfarin to 37.5 mg/week for INR 1.7.
Usage notes
Nearly all US states permit some form of CDTM, but the scope varies widely. Pharmacist-managed anticoagulation and hypertension clinics have shown outcomes at least as good as usual care. The written document governing it is often called a collaborative practice agreement.
