Enzyme produced mainly by the liver's bile ducts and by bone-forming cells, with smaller amounts from placenta and intestine. A typical adult range is about 40 to 130 U/L; high levels suggest cholestasis or bone disease such as Paget disease, healing fractures or bone metastases.
Example: ALP 410 with a normal GGT; bone source likely, checking vitamin D and a bone scan.
Commonly spoken as "alk phos". Children and adolescents normally have two to three times adult values because their bones are growing, and ALP rises in late pregnancy from the placenta. A raised GGT or 5'-nucleotidase confirms a liver source. Abnormally low ALP occurs in the rare inherited disorder hypophosphatasia.
