SOAPIE stands for Subjective, Objective, Assessment, Plan, Intervention, Evaluation, a medical documentation abbreviation.
Nursing variant of the SOAP note that adds the care actually given and the patient's response to it. It suits nursing documentation, where actions and their effects must be recorded, not just planned.
Example: SOAPIE entry: I repositioned and gave oxycodone 5 mg; E pain 3/10 after 45 minutes.
Usage notes
A further version, SOAPIER, adds R for revision of the care plan. Nursing schools often teach SOAPIE alongside DAR and PIE charting so students can adapt to different hospital systems. Electronic flowsheets have reduced narrative SOAPIE notes in many hospitals.
