s/p stands for Status post, a nursing abbreviation.

Indicates that a patient has already undergone a procedure or experienced an event, usually followed by what happened and when. It frames the current condition in light of a past surgery, injury or treatment.

Example: 72 y/o female s/p right total hip arthroplasty POD 2, ambulating with walker.

Usage notes

Read aloud as "status post". Typical forms include "s/p MVC", "s/p fall" and "s/p CABG x3 (2019)". Adding a date or postoperative day avoids ambiguity, since "s/p appendectomy" alone does not say whether surgery was yesterday or decades ago. It is not on the ISMP error-prone list, though some facilities discourage slash abbreviations in general.
