Environmental mycobacteria other than the tuberculosis complex and leprosy bacillus, found in soil and water, that cause chronic lung infections in people with bronchiectasis, COPD or cystic fibrosis, as well as skin and soft-tissue infections. Mycobacterium avium complex and M. abscessus are the most common pathogens.
Example: Nodular bronchiectasis with two sputum cultures positive for NTM (M. avium); discuss treatment versus observation.
More than 190 species have been described, and lung disease from them is rising in many countries. Unlike TB, NTM are not spread between people, and isolation is not required. Diagnosis needs symptoms, imaging and repeated positive cultures, because a single positive sample may reflect contamination. A 2020 international guideline covers pulmonary disease.
