1. Left atrial enlargement

Dilation of the left atrium, commonly from mitral valve disease, hypertension, diastolic dysfunction or atrial fibrillation. Echocardiography measures it as left atrial volume indexed to body surface area, with values above 34 mL/m2 considered enlarged; the ECG may show a broad, notched P wave.
Example: Echo: moderate LAE (LA volume index 45 mL/m2) and grade II diastolic dysfunction.
The notched P wave in lead II is sometimes called "P mitrale" because of its association with mitral stenosis. A large left atrium predicts atrial fibrillation and lowers the chance that rhythm control will succeed. Right atrial enlargement (RAE) produces a tall, peaked "P pulmonale" instead.
