HAC stands for Hospital-acquired condition, a hospital abbreviation.
Serious, potentially preventable complication that develops during a hospital stay, such as falls with injury, stage 3 or 4 pressure injuries, catheter infections or wrong blood transfusion. Medicare does not pay extra for them.
Example: Fall with hip fracture on day 3 reported as HAC; root cause analysis scheduled.
Usage notes
Under the Affordable Care Act's HAC Reduction Program, the worst-performing quarter of hospitals receive a 1 percent Medicare payment cut. Present-on-admission coding separates hospital-acquired from pre-existing conditions. Patient safety indicators feed into the program score.
