DBili stands for Direct bilirubin, a laboratory test abbreviation.
Water-soluble, conjugated form of bilirubin processed by the liver, normally no more than about 0.3 mg/dL. A high direct fraction points to hepatocellular disease or biliary obstruction, while a high total with normal direct bilirubin suggests hemolysis or Gilbert syndrome; indirect bilirubin is total minus direct.
Example: DBili 4.1 of a total 5.2: conjugated hyperbilirubinemia, imaging ordered.
Usage notes
"Direct" comes from the van den Bergh reaction described in 1913: conjugated bilirubin reacts directly with the diazo reagent, while unconjugated bilirubin needs an accelerator. In infants, a raised direct bilirubin is never normal and prompts urgent evaluation for biliary atresia. Also written D bili or conjugated bilirubin.
