CMS-1500 stands for CMS-1500 claim form, a health insurance abbreviation.
Standard paper claim form used by physicians, other professionals and suppliers to bill Medicare, Medicaid and most private insurers for professional services. Its electronic equivalent is the X12 837P transaction.
Example: Paper CMS-1500 printed in red ink for the small out-of-state plan.
Usage notes
Maintained by the National Uniform Claim Committee and formerly called the HCFA-1500. The official form is printed in a special red ink so scanners can read it, and photocopies are generally rejected. Most claims are now submitted electronically, and institutional providers use the UB-04 instead.
