A/P stands for Assessment and plan, a medical documentation abbreviation.
Final section of a progress note or H&P containing the clinician's interpretation of the problems and what will be done for each. It is the part most often read by other team members.
Example: A/P: 1. Sepsis from UTI, improving. Continue ceftriaxone. 2. AKI, resolving. Hold lisinopril.
Usage notes
Also written A&P, which in education means anatomy and physiology. Problem-based A/P sections list each issue separately. Some clinicians lead notes with the A/P (APSO order) so it is seen first. Billing is largely driven by the decision making documented here.
