q4-6h stands for Every 4 to 6 hours, a prescription abbreviation.
Range-interval order that lets the nurse give a PRN dose as often as every 4 hours or as seldom as every 6, usually for pain or fever. Hospital policies define how to interpret dose and interval ranges so that the order stays safe.
Example: Morphine 2-4 mg IV q4-6h PRN severe pain; reassess sedation in 30 minutes.
Usage notes
Range orders are allowed by CMS and The Joint Commission only when the facility has a written policy explaining how nurses choose within the range. Pain management societies support them for flexibility but warn against ranges that are too wide, such as a fourfold dose spread. Also written q4-6hr or Q4-6H.
