OCB stands for Oligoclonal bands, a neurology abbreviation.
Distinct bands of immunoglobulin G found on electrophoresis of cerebrospinal fluid but not in matched blood, showing antibody production within the central nervous system. They are present in more than 90 percent of people with multiple sclerosis.
Example: CSF positive for 7 OCB not present in serum, with an elevated IgG index; supports MS.
Usage notes
The 2017 McDonald criteria allow CSF-specific OCBs to substitute for dissemination in time, enabling an earlier MS diagnosis after a first episode. Bands are also found in infections, autoimmune encephalitis and other inflammatory conditions, so they are not specific to MS. Testing requires paired CSF and serum samples analyzed by isoelectric focusing.
